By signing the Company’s Order Form, Customer affirmatively consents to the Terms of Service, which is comprised of the following documents, including but not limited to:

• General Terms and Conditions
• Business Associate Agreement
• Data Use Agreement
• MIPS Services Schedule
• Cost Services Schedule

These documents collectively form the Terms of Service, which govern the use of Company’s software and services. No further acceptance or signature is required by Company to enforce these Terms of Service.

By signing the Company’s Order Form, you represent that you have the authority and right to bind your organization (“Customer”) to these terms and conditions.
